Hey Minnesota, this is your chance to get $100 to keep yourself, your friends and family, and your community safe from COVID-19! Minnesotans 12 years of age and older who receive their first dose between Friday, July 30 and Sunday, August 22 will be eligible for a $100 Visa gift card to spend however they choose. All you have to do is roll up your sleeves!

The $100 reward request form is open from Wednesday, August 4 through August 22. Reward requests must be submitted to the Minnesota Department of Health by 11:59 p.m. on Sunday, August 22, 2021. Vaccine information will be subject to verification by the Department of Health. Note: For security reasons, Visa cards cannot be delivered to PO Boxes. Please submit a physical address or a place where you can receive mail. It may take up to five weeks to receive your Visa card. As fall approaches and kids go back to school, getting vaccinated can help slow the spread of the highly-transmissible Delta variant and keep our communities safe.
